Inference-based CBT is an evidenced based treatment for Obsessive Compulsive Disorder. It focuses on obsessional doubt and misapplied reasoning, helping the OCD sufferer understand how their OCD is constructed, and how to resolve it. The treatment has been researched for over 25 years and was created by Frederick Aardema, PhD and the late Kieron O'Connor, PhD.

In I-CBT obsessions are not random, but are conclusions (or inferences) that the OCD sufferer comes to through the process of inferential confusion. This is a complex process that includes an over reliance on possibility, distrust of the self and senses, misapplied reasoning, the vulnerable self theme and the OCD story. I-CBT does not require practicing exposures, and the focus is on the doubt. When the doubt is resolved the anxiety and compulsion no longer exist.
